Overview
A growing behavioral health organization in Brooklyn is seeking a full-time CFTSS Program Coordinator to support and expand its HCBS/CFTSS services.
This role is ideal for a motivated, energetic professional who thrives in a fast-paced environment and wants to play a meaningful role in building and strengthening a program that serves families.
We are looking for someone who brings initiative, professionalism, and strong follow-through to daily operations.
Key Responsibilities
Manage intake and onboarding of new CFTSS referrals
Ensure timely communication with families and providers
Coordinate services efficiently and accurately
Maintain compliance documentation and tracking systems
Support outreach efforts and relationship-building with referral sources
Identify opportunities to improve processes and responsiveness
Serve as a consistent, reliable presence within the department
Assist in program growth initiatives
